Due Significance Need be given to Pancreaticobiliary Maljunction in Pediatric Practice

چکیده

Pancreaticobiliary maljunction (PBM) is a congenital anomaly defined as junction of the pancreatic and bile ducts located outside the duodenal wall beyond the influence of the sphincter of Oddi, usually forming a markedly long common channel. Other names are “anomalous arrangement of pancreaticobiliary ducts” or “anomalous union of biliopancreatic ducts” [1]. Average length of common channel is 4.4mm [2]. Some authors defined PBM as common channel more than 5 mm [3].

Pancreaticobiliary maljunction without bile duct dilatation in children: distinction from choledochal cyst.

OBJECTIVES Pancreaticobiliary maljunction without bile duct dilatation (nondilated type) is rare in children, and its definition remains unclear. There is controversy over treatment between pediatric and adult patients. We reevaluated our previous definition of the nondilated type in children (common bile duct diameter <or=6 mm on cholangiography). PATIENTS AND METHODS Of 150 children with pa...

Recurrent Cholangitis with Congenital Hepatic Fibrosis and Pancreaticobiliary Maljunction after Roux-en-Y Reconstruction

A 1-year-old girl had pancreaticobiliary maljunction, a choledochal cyst, and polycystic kidney. At the age of 4 years, she was treated by resection of the choledochal cyst and Roux-en-Y reconstruction because of the cyst's risk of cancer. She was diagnosed as having congenital hepatic fibrosis based on the histological findings. Postoperatively, she suffered recurrent fever of unknown origin, ...

Annular pancreas concurrent with pancreaticobiliary maljunction presented with symptoms until adult age: case report with comparative data on pediatric cases

BACKGROUND Annular pancreas (AP) concurrent with pancreaticobiliary maljunction (PBMJ), an unusual coexisted congenital anomaly, often presented symptoms and subjected surgical treatment at the early age of life. We reported the first adult case of concurrent AP with PBMJ presented with symptoms until his twenties, and performed a literature review to analyze the clinicopathological features of...

Pitfalls of MRCP in the diagnosis of pancreaticobiliary maljunction.

CONTEXT Magnetic resonance cholangiopancreatography (MRCP) is useful for examining the pancreatic duct system in patients with acute pancreatitis instead of using endoscopic retrograde cholangiopancreatography (ERCP), as ERCP-induced pancreatitis represents a serious problem. However, we present here a case of idiopathic acute pancreatitis in which MRCP suggested pancreaticobiliary maljunction,...

Altered expression levels of occludin, claudin-1 and myosin light chain kinase in the common bile duct of pediatric patients with pancreaticobiliary maljunction

BACKGROUND In pancreaticobiliary maljunction (PBM), the sphincter of Oddi can not control bile and pancreatic juice flow, which may lead to two-way reflux of bile and pancreatic juice, thus causing chronic inflammation, thickening, fibrosis and metaplasia of the common bile duct wall. These pathophysiological changes have been linked to disruption of the epithelium barrier in the common bile du...
